Need to Upload or Replace a Physical?
Sports physicals are uploaded through Annasis, not on this website.
- 1Log in to your Annasis account and open the Parent Portal.
- 2Select “Menu.”
- 3Scroll all the way down and select “Student File Requests.”
- 4Choose your child in the “Select a Student” dropdown at the top of the page (you will see only your own children), and confirm the school year is set to the current year.
- 5Find the “Physical Examination” file request and select “Update” to upload the document.
- 6Upload the page showing your child’s name, examination date, sports-clearance decision, and the medical provider’s signature.
Important
A medical-history questionnaire or the first page of a physical form by itself will not be accepted. We need the page containing the provider’s clearance decision, signature, and examination date.
What counts as a valid sports physical?
The Oaks provides a blank sports physical form that families may take to their medical provider. You are not required to use The Oaks form. We can also accept a sports physical form from a medical provider, clinic, school district, or another organization, provided it contains all required information.
To be accepted, the document must clearly include:
- The student’s full name
- The date of the physical examination
- A statement or checked box showing whether the student is cleared for sports participation
- Any restrictions or conditions placed on participation
- The medical provider’s signature
- The medical provider’s name or identifying information
A medical-history questionnaire or the first page of a physical form by itself is not sufficient. We must receive the page showing the provider’s sports-clearance decision, signature, and examination date.
If your form has multiple pages, you do not need to upload medical-history pages that are intended to remain with the provider. However, please make sure the uploaded document identifies the student and contains all of the information listed above.
